What the inspector wrote

On 8/19/25, Licensing Program Analyst (LPA) Crystal Ali met with Director to conduct an unannounced case management (CM) inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received on 8/15/25. Incident occurred on 8/13/25, child was running in between bathroom doors in the classroom and bumped his right ear on the metal door jam lock area. The tip of right ear was bleeding and staff utilize first aid procedures. Parent arrived approximately five minutes after incident occurred and took child to urgent care. The nurse practitioner used glue to close the cut and placed bandage over it. Child returned to daycare facility on 8/14/25. Upon arrival, LPA observed 70 preschool and 7 teachers providing supervision and care. During this inspection LPA conducted private interviews with child, director, teacher and parent (over the phone). In addition, LPA completed a safety inspection of the facility classroom Sunflower. During the inspection, LPA took picture of bathroom and child\342\200\231s injury. CM findings show that the facility followed Title 22 and Health and Safety Codes in reporting incident to required parties, providing care and supervision. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Director, Ratna Rajaratnam.
