Follow-up on corrections·September 26, 2025

On September 26, 2025, Licensing Program Analyst (LPA) Isabel Ortega met with Facility Program Administrative Staff to conduct an unannounced case management inspection. The purpose of this case management is to follow up on a self reported unusual incident report (UIR) submitted to the Department on 06/16/2025. The unusual incident report is regarding staff #1. Upon arrival, there were 49 children observed in care and 21 staff proving care and supervision. According to facility Staff placed by Human Resource on Administrative leave is no longer employed by Primeros Pasos Child development Center and staff have attended a training on Personal Rights. Therefore the Plan of Correction (POC) has been met. No citations were issued, facility followed Title 22 regulations and completed the plan of correction within time frame. Facility has been cleared by visit inspection, today. A notice of site visit was provided and requested to be posted for 30 days. An exit interview was conducted, a copy of this report, notice of site visit and appeal rights were provided to facility.
