What the inspector wrote

On 05/08/2025, Licensing Program Analysts (LPAs) Elizabeth Martinez and Octavia Nolan conducted a case management inspection. LPA met with licensee, Elida Cruz. LPA took a census and discussed the reason for the visit. Licensed rooms are the daycare room and hallway bathroom as shown on the facility sketch. The backyard is accessible to children in care. Licensee provides care to infants and adheres to required Safe Sleep regulations. The purpose of today\342\200\231s inspection is to receive an unusual incident report involving Child #1 (C1). On 04/16/2025, four children were playing in the backyard. C1 was playing on Child #2\342\200\231s (C2) back when she fell to the ground on her elbows. Licensee administered first aid and applied an ice pack to the child\342\200\231s elbow. C1 continued to have pain in the elbow and the Licensee called the child\342\200\231s authorized representative. The Licensee was notified on 04/21/2025 that the child was taken to Urgent Care and was treated for a fracture with an arm cast. LPA inspected the area where the incident took place, conducted a file review, and interviews. There are no hazards present in the backyard and there is padding where the child fell. This appears to be an isolated incident and the Licensee took appropriate measures to address the child\342\200\231s injury. The Licensee also followed appropriate reporting requirements required by the Department. Exit interview conducted and report was reviewed with Licensee, Elida Cruz. Appeal rights were provided. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
