What the inspector wrote

On 08/13/2025, a case management visit is conducted by Licensing Program Analyst (LPA) Lady Cabrera and met with Licensee Adriana Ceja Gonzalez. On 06/20/2025, LPA arrived at the facility to investigate a complaint, it was confirmed the facility did not report the incident to Fresno Community Care Licensing (CCL) by telephone or fax within the CCL’s next working day and during its normal business hours. LPA provided Reporting Requirements regulation to Licensee. On 06/23/2025, Licensee submitted a late an Unusual Incident Report to the CCL. During the complaint investigation, Licensee confirmed she received a picture of Child’s 2 bruised bite mark on their left leg on 06/02/2025. Licensee confirmed children were playing with the Licensee’s dog. Per records reviewed and interviews, CCL did not receive a phone call and/or an Unusual Incident Report until 06/23/2025. Facility did not comply with the reporting requirements regulation. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited on LIC809D. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. Appeal rights were provided to Licensee.
