What Happened

Family child care home has significant safety and compliance violations. Licensee was initially the sole adult caring for 11 children (citation issued). Multiple physical hazards identified including exposed nails in damaged walls and broken play equipment. Children's immunization records incomplete and infant sleep plan missing.

Observations

  • Licensee was sole adult providing care to 11 children at inspection start (10 aged 2-5 years, 1 infant under 24 months) - citation issued
  • Kitchen drawers contained scissors and screwdrivers (removed during inspection)
  • Loose screws in TV stand drawer in living room (removed during inspection)
  • Wall damage in day care room exposing metal rounded corner
  • Diaper changing table with stains, crumbs, and torn changing mat
  • Excess toys and children's furniture stacked creating clutter in day care room
  • Kitchen gate normally securing kitchen/dining/living room areas was not up during inspection
  • Children's files missing immunization documentation for Measles
  • One infant missing required Infant Sleep Plan form (LIC 9227)
  • Outdoor play area to be made inaccessible due to planned renovations
  • Missing balusters on stairwell requiring replacement
  • Licensee's Mandated Reporter Training expired (07/19/2023)
  • Licensee's CPR/First Aid certification expires 05/12/2025
