What the inspector wrote

On March 21, 2025 FAS was down and unavailable. During the inspection, LPA created a Word document. The following is an electronic copy of the report. This report will be sent to the licensee. On March 21, 2025, LPA Elizabeth Martinez and LPM Cynthia Brannon conducted a case management inspection. LPA met with licensee, Mirian Hernandez. During the inspection, LPA observed six children, with licensee infants and 3 older children. Per licensee, there are no weapons, ammunition, or bodies of water. There is a trampoline that is kept inaccessible to children in care. Hours of operation is Monday thru Friday from 5:00 AM to 5:00 PM. Licensee provided her contact phone number (661) 229-6808. Licensed rooms are the daycare room and hallway bathroom as shown on the facility sketch. The backyard is accessible to children in care. Per licensee, she does not have dogs or cats. Licensee provides care to infants and adheres to required Safe Sleep regulations. The purpose of today’s inspection is to receive an unusual incident report involving C1. During today’s inspection, an interview was conducted with the licensee. Documents were provided during today’s inspection. The interview reflects that the licensee followed her training and protocol. Licensee conducts well checks on children upon arrival, throughout the day, and when children leave the facility. Exit interview conducted with licensee, Mirian Hernandez. No deficiency was cited during today’s inspection. This report shall be made available to the public upon request. A Notice of Site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00.
