On April 17, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management incident inspection. LPA met with Licensee Mirian Hernandez. LPA toured the inside and outside of the facility, and a census was taken. The purpose of the inspection was to follow up on the unusual incident report submitted to the Fresno Regional Office on March 18, 2026. On March 17, 2026, at approximately 2:00 p.m., Licensee reported that Child #1 (C1) was placed on the carpet after nap time while children were putting away blankets and mats. Licensee stated that Child #2 (C2) tripped on their blanket and fell onto C1, causing C1’s body to bend to the right. C1 immediately began crying, and Licensee observed that C1’s right leg appeared twisted and that C1 did not want to move it. Licensee stated she attempted to contact C1’s mother, who returned the call around 3:00 p.m. and picked up C1 shortly after. C1 was taken to urgent care, where X-rays later confirmed a fracture of the right femur. C1’s mother notified Licensee of the diagnosis at approximately 6:50 p.m. Licensee stated that C2 was not injured and that C2’s parents were not notified. During today’s inspection, LPA conducted file reviews, observations, obtain facility documents, and staff interview. Based on information gathered, LPA determined that the incident was isolated, children’s personal rights were not violated, and Licensee maintained supervision at the time of the incident. An exit interview was conducted, and the report was reviewed with Licensee Mirian Hernandez. Per Title 22, Division 12, Chapter 3 of the California Code of Regulations, no deficiencies are cited. Licensee was provided appeal rights. This report shall be made available to the public upon request. The LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
