What the inspector wrote

On 05/08/2025, Licensing Program Analysts (LPAs) Elizabeth Martinez and Octavia Nolan conducted an unannounced case management inspection and met with Licensee, Brenda Alcazar. LPAs toured the facility inside and outside and took a census. On 04/09/2025, the Licensee notified Community Care Licensing (CCL) via telephone regarding an Unusual Incident. On 04/08/2025, Child #1 (C1) pushed Child #2 (C2) in the living room causing the child to fall. C2 hit their head on the children's table resulting in a cut above his right eyebrow. The Licensee administered first aid by applying pressure to stop the bleeding and cleaned the wound. The Licensee notified the parent of the incident, and the child was picked up at 3:20 PM. The parent notified the Licensee that the child was taken to the emergency room later that day and was treated with topical antibiotics and a bandage. The child returned to care on 04/09/2025 and is doing well. LPAs inspected the area where the incident took place and there are no hazards present. LPAs conducted a record review and interviews. LPAs also discussed how to handle challenging behaviors with Licensee. This appears to be an isolated incident, and the Licensee took appropriate measures to address the child's injury and followed appropriate reporting requirements required by the Department. Exit interview conducted and report was reviewed with Licensee, Brenda Alcazar. Appeal rights were provided. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
