On 4/5/2023, Licensing Program Analyst (LPA) Carol Heath conducted an unannounced Required 1 Year inspection at the Horton Family Child Care. Upon arrival, the LPA rang the doorbell several times and heard the children were inside. LPA called the licensee, Sharyon for several times. Nobody answered the phone. 10 minutes later, a car parked on the street and walked into the house. LPA met with the licensee\342\200\231s daughter, Amber Horton who guided the LPA on a tour of the facility. According to Amber, her mother has family emergency, and she is not here. LPA observed 12 children [3 infants: (1) 4 mons and (2) 16 months, 3 toddlers, 4 preschoolers, 1 is 5 years old and 1 child is 6 years old) with 1 assistant (Ashley W.) along with children before the licensee\342\200\231s daughter arrived. Family members residing in the home include 4 adults (applicant, applicant's spouse, and applicant's two adult grandchildren). Per LIS, facility annual fees are current. All adults living in the home have been background cleared. Per licensee, the hours of operation are Monday through Friday 6:00 a.m. to 6:00 p.m. Incidental Medical Services (IMS) were discussed. The licensee will not provide IMS service at this time. The Home is set up as follows: This is a two story 6 bedroom, 3 bathroom home with kitchen/dining, family room, living room, playroom, laundry room, and garage. There is a spa, and body of water on the premises. The garage is used for storage only and no childcare activities are conducted there. The home was inspected for safety, comfort, cleanliness, telephone service, central air, and heat and ventilation. The home has central heating and air conditioning. All windows have screens and are free of cracks, bugs, and debris. Main Area: Main care is provided in the family room (located next to the kitchen). Children use the bathroom in the back bedroom used as the sick room Children have access to the kitchen and backyard. Family rooms: In the areas, LPA observed a fireplace that was properly screened via a mirror glass barrier which made it inaccessible to children. LPA observed age-appropriate toys and furniture for the children. LPA observed: A small table was observed with a total of eight chairs. Several plastic storage bins were observed in which games and toys are stored for the children. A small play kitchen was located by the door with which children can play. There are games and books on the premises of this facility. There are mats on the floor that have educational/learning activities on them. Children Bathroom (#1): Children use bathroom #1 in the back bedroom #1 used as the sick room. The bedroom #1 is through the kitchen (off-limit). The Bathroom was toured, and inspected sink/toilet is in operable condition. Toilet and faucets are clean, safe, and operable. LPA observed some toothbrushes and oil in the cabinet. The bathroom was clean, sanitized, and in good repair. Backyard (Declaration): The backyard was inspected; The backyard is gated all around. According to the licensee\342\200\231s daughter, the Children do not play in the backyard due to the weather. The backyard is completely fenced. There are age-appropriate toys and play structures in good repair available to children. 3 small dogs and 1 big dog stay in the two separate off-limits dog runs in the backyard. The licensee advised to keep dogs in dog run during daycare hours and maintain the cleanliness of backyard free of animal excrement. LPA observed a fence around the charcoal grill to make it inaccessible to children. \303\230 Off-limit : Off-limit areas include the entire upstairs (Bedroom #2 through #6, bathrooms #2 and #3). LPA reminded the licensee to keep the safety gate on during the operation hours, laundry room, living room, playroom, and garage. LPA reminded the licensee to put the safety gate on the stairs. Kitchen/Dining Room: The kitchen was inspected to ensure hazardous and dangerous items were inaccessible to children (Safety latches). In the kitchen, all sharp utensils, poisons, and medications are made inaccessible to children with child safety latches on cabinet doors and drawers. Sharp knives are kept in a butcher block in the gated kitchen. LPA observed safety locks on lower kitchen cabinets to make them inaccessible to children. LPA observed several medications on the kitchen counter. The licensee\342\200\231s daughter was able to remove them from the area. \302\267 AC/Heating Unit / Swamp Cooler unit was observed. AC/Heating Unit is located on the right side of the home and is inaccessible to children via barrels blocking access to the AC unit. \302\267 Bodies of water: Pre the licensee, there were bodies of water in the home (SPA). Fences are at least five feet high and shall be constructed so that the fence does not obscure the pool from view. The bottom and sides of the fence shall comply with Division 1, Appendix Chapter 4 of the 1994 Uniform Building Code. PA observed the barriers; gates shall swing away from the pool, self-close, and have a self-latching device no more than six inches from the top. Pool covers shall be strong enough to completely support the weight of an adult and shall be placed on the pool and locked while the pool is not in use. \302\267 Electrical outlets: All unused electrical outlets are plugged in and made inaccessible to children. \302\267 Food: The licensee is enrolled in Food program. The licensee will provide Breakfast, lunch, and snacks. \302\267 Fire extinguisher (2A10BC) : LPA observed a required fire extinguisher (2A10BC) reading in Green and located in the kitchen, inaccessible to children. It meets standards established by the State Fire Marshall. \302\267 Fireplace: The fireplace was observed in the living room and is screened to make it inaccessible to the children. \302\267 Hanging window blind cords : The cords are inaccessible to children. \302\267 Isolation area (Illness): Per the licensee, if the child shows signs of illness, they will be separated from other children and stay in Living room. \302\267 Medications and cleaning solutions : Detergents/cleaning compounds are in the upper kitchen cabinet, inaccessible to the children. Medications are in the off-limits bedroom. \302\267 Napping: Children will nap in designated areas with adult supervision. LPA observed 14 mats in the closet. \302\267 Overnight Care: According to the licensee does not provide overnight care. \302\267 Pets: There are 3 small dogs (outdoor) and 1 big dog . The licensee will need to email LPA the current vaccinations. \302\267 Phone service: There is a working landline or cell phone \302\267 Smoke Detectors and Carbon Monoxide: The smoke detectors and carbon monoxide devices tested operable. \302\267 The First Aid kit is in the key-locked closet, inaccessible to children. The First Aid Kit was observed to be complete with supplies and a first aid manual . \302\267 Transportation : The licensee does provide transportation for children. The licensee has a valid California driver's license, valid vehicle insurance, and vehicle registration. \302\267 Weapons or Firearms: Per the licensee, there are No Firearms at the facility at this time. LPA does not observe any firearms. Documentation: \302\267 Child files : LPA observed that 6 children's files contained all required licensing documents. \302\267 CPR/First Aid: LPA observed licensee has current Pediatric CPR and First Aid Training with an expiration date (of 11/2022) . \302\267 Fire Drill and Disaster Drill: Per the licensee, fire, and disaster drills are conducted every 6 months; the last drill was documented and performed on 02/14/2023 . \302\267 Immunization: The licensee and her assistant have the required immunizations (MMR and DTaP). The licensee and her assistant provided a written statement declining the influenza vaccination. \302\267 Infant Sleeping Plan (LIC 9227) and Sleeping Log: There are 3 infants enrolled in the facility. The licensee did not complete Sleeping log in the files. LPA shared the Safe Sleeping Regulation with the licensee. \302\267 The licensee post all required information \302\267 Mandated Reporter Training: The licensee has completed and renewed the online mandated reporter training at www.mandatedreporterca.com on 11/2022, The licensee will need to renew the training \302\267 Staff Personnel File: LPA observed 2 staff information. The file contained all required licensing documents \303\230 Documentation: \302\267 Child files : LPA reviewed 6 children's records, the records are complete and missing forms. \302\267 CPR/First Aid : LPA observed licensee has current Pediatric CPR and First Aid Training with expiration date (11/2022) 1 hour of nutrition training, (8) hours of Preventive Health and Safety Training. \302\267 Facility Roster: LPA observed Child Care, Facility Roster. Per Licensing Information System, facility annual fees were current. \302\267 Fire Drill and Disaster Drill : Per the licensee, fire and disaster drills are conducted every 6 months; the last drill was documented and conducted on 01/28/2022. \302\267 Immunization: The licensee has the required immunizations. The licensee provided a written statement declining the influenza vaccination. \302\267 Infant Sleeping Plan (LIC 9227): LPA observed 1 LIC 9227 in the child file. The infant is 7 months old. \302\267 Licensee has posted as required the Facility License , Emergency Disaster plan , and Parents Rights Poster. The facility roster is current. \302\267 Mandated Reporter Training : The licensee has completed the online mandated reporter training at www.mandatedreporterca.com , and will renew 11/18/22 \302\267 Staff Personnel File: LPA observed 1 staff information. The file contained all required licensing documents The following information was discussed with the licensee : \303\274 Mandatory Forms for the children's files and provider's files. \303\274 The licensee is reminded that 100% supervision is required for children at all times. \303\274 Capacity requirements, Roster requirements, and Documentation requirements for disaster drills (fire and earthquake). \303\274 Licensee was made aware that it is their responsibility to know the regulations and anyone who assists in providing care. Licensee was advised that the inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility's phone number; if the phone number is changed, licensing must be notified. \303\274 Licensee was advised of the requirement to report unusual incidents and injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B \303\274 The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hotline at 1-800-540-4000. Also, call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B). \303\274 Criminal Record Statement: Licensee [or facility representative] was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption or transfer their existing support or exemption prior to the initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. \303\274 Safe Sleep: LPA discussed the safe sleep regulations with the licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. \303\274 A baby walker shall not be allowed on the premises of a family child care home in accordance with Health and Safety code sections 1596.848(b) and (c). State law prohibits baby walkers, bouncy seats, exersaucer, and other items that fall into that category. \303\274 Notice of Site Visit: A notice of site visit was given and must remain posted for 30 days. \303\274 Posting Requirements: Failure to comply with posting requirements shall result in an immediate civil penalty of $100. \303\274 The regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). \303\274 Licensee was advised to visit the CCL website ( www.ccld.ca.gov ) to obtain updates on courses and updates/changes to the regulations. n Our Quarterly updates come out every 3 months.
