What the inspector wrote

On 1/29/2025 Licensing Program Analyst (LPA) Isabel Ortega conducted a pre-licensing relocation inspection. Licensee is requesting a relocation from a large Family Childcare License number 157700024 to new License number with capacity of 14 children. LPA announced the purpose of the inspection and was granted entry by Licensee. A tour of the facility was completed. Upon arrival LPA observe one child in care. Licensee was initially licensed on 1/19/2023 as a Small Family Childcare Home. The Fire Department has inspected the home and granted a fire clearance for 14 children on 1/09/2025. Family Childcare Home will operate Monday through Saturday from 4:00 a.m. to 4:00 p.m. The facility plans to continue to participate in the food nutrition program and will provides breakfast, morning snacks, lunch, afternoon snack and dinner as needed. This is a single-story home with three bedrooms, Family room, three restrooms, laundry room and attached garage. Main care will be provided in the family room referred to as the childcare day care area. Children will utilize the restroom located in the Family Room. LPA observed a door alarm on door that leads to the outdoors. The home has telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. There are age-appropriate toys and equipment on the premises. According to Licensee there are no bodies of water on the premises. LPA did not observe any bodies of water during inspection. Per the licensee there are no weapons or firearms of any kind in the facility, nor did the LPA observe any weapons during inspection. LPA observed cots for napping and individual playpens will be provided for infants during nap time. The backyard will be utilized by the children for outside play and back yard is fenced all around. LPA observed a chain gate with white panel strands and gate key locked. The outdoor play area was inspected and play equipment was observed to be free of hazards, loose and sharp parts. LPA observed age-appropriate toys both inside and outside of the home. The First Aid kit with a temperature thermometer was observed and complete. The required fire extinguisher (2A10BC) is reading in green (service once a year LPA observed a yellow tag with date 7/03/2024). Smoke and carbon monoxide detector is operating condition. Fire and disaster drills shall be conducted every six-months, last emergency Fire and earthquake drill was documented on 10/09/2025 at 5:00 p.m. Licensee has all the required posted documents: Facility License (LIC 203, Notice of Parent's Rights Poster (PUB 394), Emergency Disaster Plan (LIC 610A), and Earthquake Preparedness Checklist (LIC 9148). The licensee and staff maintain proof of immunization against pertussis (TDAP), measles (MMR), Tuberculous (TB) and written statement opt out of influenza. Licensee provided certificate of completion Pediatric First Aid and CPR training which does not expire until 03/30/2025. Licensee is aware Pediatric CPR will expire in March shall be renewed. The Preventative Health and Safety training with Nutrition and Lead is dated 1/09/2025. The facility annual fees are current. The parent board was reviewed and had all the required forms posted and visible to parents. Licensee was reminded with a capacity of 14 children, a Qualified Assistant shall be present whenever more than 8 children are present in care. Licensee was provided with a capacity and ratio handout pertaining to large Family Child Care Homes and the various age groups that can be under care at one given time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at http://www.ada.gov/childqanda.htm Licensee was reminded with a capacity increase she must have a qualified assistant present whenever she has more than 8 children in care. Licensee was provided with a capacity and ratio handout pertaining to large Family Child Care Homes and the various age groups that can be under care at one given time. The following was also discussed with the licensee: 1. The following items are zero tolerance by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both 2. Pediatric First Aid and CPR: American Heart Association or American Red Cross or Emergency Medical Services Authority (EMSA) approved in Pediatric First Aid and CPR must be present. Certification must be renewed every two years. 3. Licensee was informed that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com 4. In the absence of the licensee a qualified adult must be present, supervising the children; a qualified adult is an individual who has a valid and current Pediatric first aid/ CPR-adult-child- infant certification (EMSA approved), a valid criminal record clearance associated to the facility license, immunization's (MMR, TDAP, TB and Influenza or Influenza declination), AB 1207 Child Abuse Mandated Reporter Certificate. 5. A current roster of children enrolled must be available and maintained for a period of 3 years, even after children are no longer attending the facility. 6. Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the license shall be terminated 7. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary and smoke and carbon monoxide detectors should be checked, and batteries replaced as needed. 8. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home. 9. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing (refer to LIC 624B). 10. Fire and safety drills must be performed every six (6) months and documented for review by the Department. 11. Smoking is prohibited in the family childcare home. 12. Children and staff records must be maintained and updated as needed and be available for review by the Department. 13. Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The licensee and all adults working with children have proof of immunization. 14. Inspection Authority: All adults living and working in the home shall be made of aware of the Department\342\200\231s right to inspection the home, which includes, but is not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation. 15. The facility license number must be on all advertisements, publications, or announcements with the intent to attract clients. 16. Isolation for children: When a child is ill, he/she shall be separated from other children (reference 102417(e) Operation of a Family Childcare Home). 17. Liability Insurance was discussed; LPA advised applicant to review Title 22 Regulation 102417(m)(1) for additional information. 18. Dog(s) and/or pets are recommended to be isolated from children in care. 19. No baby bouncers, no infant walkers, no Johnny jumpers, no saucer chairs, and any other item that falls into this category is not permitted in the facility. Safe Sleep regulations (under 24 months) were discussed with Licensee and referred to the CCL web site for additional information and PINS. Infants under 2 years of age shall have the 15-minute checks logs completed during nap time. All infants under one year of age shall have the LIC9227 sleep plan completed by parent until infant master\342\200\231s developmental stage of rolling over on their own. LPA also informed licensees of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. MyChildCarePlan.org- Family Childcare Homes Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Megan\342\200\231s Law - Family Childcare Homes During the exit interview , according to licensee there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot line at 1-800-540-4000. Licensee has met Title 22 regulations; Fire clearance was granted 01/09/2025, therefore, a Large Family Childcare Home License capacity of 14 children will be granted effective 1/29/2025. An exit interview was conducted, and a copy of this report, appeal and notice of site visit was provided to Licensee on this day. All Licensing reports are recommended to be kept on file for a minimum of three years. Notice of site visit (NOA) shall be posted for 30 days from today.
