What the inspector wrote

On 4/22/25, Licensing Program Analyst (LPA) Crystal Ali met with Site Supervisor Teresa White to conduct an unannounced case management inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received 4/8/25. Incident occurred on 4/7/25, child had seizure at facility, 911 was called and child was transported to hospital via ambulance. Upon arrival, LPA observed 1 infant, 11 toddlers, 48 preschool and 10 staff (including 1 cook and 1 janitor) providing care and supervision. During this inspection LPA conducted interview with site supervisor and observed the child in question. In addition, LPA completed a safety inspection of the facility grounds and obtained copy LIC 500, and copies of documentation regarding child health history. Due to the need to gather additional information, the case management will require further investigation. A notice of site visit was given and must remain posted for 30 days.     (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
