What the inspector wrote

On 7/2/25, Licensing Program Analyst (LPA) Crystal Ali met with Director Denise Campos to conduct an unannounced case management inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received 6/19/25. Incident occurred on 6/18/25, child mother reported child was taken to doctor due to arm pain. Doctor reported a sprained wrist. Police arrived to 6/19/25 due to a complaint they received stating that following: kitchen being dirty, children biting each other, student hitting teachers, and child shoe in trash can. Upon arrival, LPA observed 41 children and 11 staff providing care and supervision. During this inspection LPA toured the facility, conducted interview with Director, child, and employee (child mother). In addition, LPA completed a safety inspection of the facility grounds and obtained copy of LIC 500, and copies of documentation regarding incident form child file, and employee file. Due to the need to gather additional information, the case management will require further investigation. A notice of site visit was given and must remain posted for 30 days.
