On 7/2/25, Licensing Program Analyst (LPA) Crystal Ali met with Director Denise Campos to conduct an unannounced case management inspection. The purpose of the case management was to provide conclusion on investigation regarding unusual incident report (UIR) received 4/8/25. Incident occurred on 4/7/25, child had seizure at facility, 911 was called and child was transported to hospital via ambulance. Upon arrival, LPA observed 41 children and 11 staff providing care and supervision. During this inspection LPA completed tour of facility and conducted interview with director and reviewed training documentation of employees. LPA obtained copy LIC 500 and copies of training documentation regarding medical treatment and responsiveness to required parties of child. No deficiencies have been cited. CM findings show that the facility followed Title 22 and Health & Safety Codes in reporting this incident to all responsible parties. Technical violation was provided to agency see LIC 9102. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Director, Denise Campos.
