What the inspector wrote

On 12/23/24, Licensing Program Analysts (LPAs) Crystal Ali and Carol Heath met with Site Supervisor Maricela Leon to conduct an unannounced case management inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received 12/16/24. Incident occurred on 12/4/24 and 12/12/24, child mother contacted the director informing her of a bruise on child chest and a bald spot on the child head. On 12/13/24, police arrived to facility in regard to incidents reported to them. Upon arrival, LPAs observed 6 infants and 2 teachers providing care and supervision. During this inspection LPAs conducted interview with site supervisor (Maricela Leon) and observed the child in question. LPA Ali called Director (Denise Campos) and left voicemail message to gain more information regarding UIR. In addition, LPAs completed a safety inspection of the facility grounds. In addition, during the inspection, LPAs obtained copies of documentation of incident reports and health checks from child #1 file. Due to the need to gather additional information, the case management will require further investigation. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Site Supervisor Maricela Leon.
