What the inspector wrote

On 7/28/25, Licensing Program Analyst (LPA) Crystal Ali met with the facility representative Maricela Leon (site supervisor), to conduct an unannounced Annual/Random inspection. LPA disclosed the purpose of the inspection and was granted entry to the facility. Upon arrival, LPA observed 2 classrooms in use with a total of 9 infants and 1 teacher and 2 teacher assistants on the premises. The hours of operation are 6:30 AM - 5:30 PM, Monday - Friday. Incidental Medical Services (IMS) were discussed. No smoking or weapons on the premises. Physical Plant: LPA toured and inspected the preschool classrooms. Furniture and equipment were checked for age appropriateness and good repair. All rooms are clean. Telephone service was verified. Heating, lighting, and ventilation are adequate. There are cubbies for infant\342\200\231s belongings labeled with their names along the main hallway. LPA observed age-appropriate toys and materials. Daily activities schedule was observed in each classroom. Facility does offer napping in their program. Napping is done in classroom #2. Safe sleep logs were observed by LPA. LPA observed cribs (6), one highchair, and 3 mats of the floor in classroom #2. Drinking water is available inside the classroom in the form of tap water into their sippy cups or bottles. In classroom #1, LPA observed 1 large play yard in the middle of the room, a small ball pit, diaper changing station right next to sink, and two cribs used for napping and isolation. The fire extinguisher (checked 10/30/24 and in the green), a smoke detector, and a carbon monoxide detector are on the premises. Infants are inspected for illnesses as they arrive with a no-touch thermometer and an overall wellness check. The classroom has a separate area to the side for isolating from other infants in the nap room (classroom #1) that has a safety gate. A review of medication policy indicated that prescription medication is administered only with the parent\342\200\231s written permission. The facility representative and other administration staff administer medication and document the dosage, date, and time on a log. Medication is brought and locked at the center (in the staff bathroom cabinet). Medication is labeled correctly and stored in its original container. Medication is kept at the center until expiration and sent home with the parent for disposal. There are currently no infants in care with IMS needs or medication. All containers used for storage of solid wastes, including moveable bins, shall have a tight-fitting cover that is kept on; they shall be leakproof and rodent-proof. Facility is not part of food program. Parents provide diapers, wipes, and formula. A Fire Drill/Earthquake log is posted in the office. The center conducted disaster drills for at least 6 months. Last completed on 6/13/25. LPA observed a first aid kit with first aid manual located in the kitchen on top of the stove cabinet. First aid kit is complete. A current child roster (LIC 9040) is available. LPA observed no bodies of water on the premises. Staffing Ration and Capacity: Upon arrival, LPA observed 2 classrooms in use with a total of 10 infants, 1 teacher and 2 assistant teachers on the premises. Transportation arrangement: Facility does not provide transportation. Lead Testing: The center had drinking water tested for lead contamination levels on 7/28/2021. The testing results showed no detection of lead in the water supply. Documentation is on file at CCL Palmdale Regional Office. The Outdoor: The play equipment was inspected for health, safety, cushioning material, good repair, and age appropriateness. The play area was inspected and found to be free of hazards and inaccessibility to bodies of water. The infant play yard is gated and separate from school age play yard. LPA observed a large, shaded area for infant play, small sand box, small play equipment, and toys. There is small gap between the fence where there is a trash can blocking infant access to school age play yard area. Facility representative was informed and stated facility will remove the trash can and properly close off the space. The parent board was reviewed and has all the required forms posted. Sign-in and out sheets were reviewed. The Teacher to child ratio was observed, and staff name were recorded. Care and supervision were evaluated to determine if the children\342\200\231s basic needs were met and appropriate. Staff Records/ Children Records: Staff and Children's records were reviewed and in compliance. LPA reviewed five employee files. LPA verified there is at least 1 staff person present with current CPR and First Aid Training. All employees have a valid Mandated Reporter Training. LPA reviewed five children's records, complete with the parent\342\200\231s signature. Facility needs to add all required items to complete needs and service plan for infants. Parents must sign this plan, and it must be revised every quarter. LPA observed the feeding plan and LIC 9227 for each infant. Kitchen/Food Service: There is a clean, fully equipped kitchen with a refrigerator, freezer, stove, dishwasher, and microwave oven. Parents provide food for their children. The facility does have some infant snacks and baby food present for infants if parents forget to provide those items. Allergy lists are posted in the kitchen on the refrigerator. No infants with food allergies. LPA observed an appropriate amount of food and snacks and staff preparing lunch (with gloves). Soaps, detergents, cleaning compounds, or similar substances are stored separately from food supplies. The chemicals are kept in the middle closet in the kitchen with safety latch. LPA observed that food preparation and storage areas are kept clean and free of litter and rubbish. LPA observed no knives and sharp items in the facility. LPA observed 9 highchairs in the kitchen and bin on counter where pacifiers are stored with the child name. Menus are posted at least one week in advance and made available for review by the parents. LPA observed the refrigerator #1 and #2. Refrigerator #1 has bins for each infant food for the day with the infant name on the bin. Refrigerator #2 is for staff only. Criminal Record Clearance - Child Care Centers Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Lead Testing \342\200\223 Child Care Centers (CCC) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Safe Sleep - Child Care Centers LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/ safe-sleep as an additional resource. LPA also informed facility of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) - Child Care Centers Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP .
