This report is amended. This report is a case management-incident report not case management-infectious disease outbreak. On October 28, 2025, Licensing Program Analyst (LPA) Crystal Ali met with Alexis Savedra (Owner), conduct an unannounced case management inspection. The purpose of the case management (CM) was to follow up on unusual incident report (UIR) received on 10/17/25. The incident occurred on 10/8/25, regarding administrative action of an employee. Upon arrival, LPA observed 6 toddlers and 8 preschoolers, 5 staff members providing care and supervision. During this inspection, LPA interviewed the Owner and toured the entire facility. CM findings show that the facility followed Title 22 and Health and Safety Codes guidelines for reporting the incident and ensuring safety of children. No deficiencies have been cited.
