What the inspector wrote

On 3/10/25, Licensing Program Analyst (LPA) Crystal Ali met with Site Supervisor Carleen Bencoma to conduct an unannounced case management inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received 2/3/25. Incident occurred on 2/3/25, child was riding the tricycle fast and tried to stop. Child obtain injury to his mouth. Mom took child to the dentist and a tooth was removed. Upon arrival, LPA observed 17 preschool and 5 teachers providing care and supervision. During this inspection LPA conducted interview with Site Supervisor, Teacher, Child, and Parent (by phone). The CM investigation concluded that teachers were following Title 22 and Health and Safety guidelines. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Site Supervisor Carleen Bencoma.
