On November 2, 2023, Licensing Program Analyst (LPA) Paul Garcia conducted an unannounced case management inspection. LPA met with Linda Kroff-Reis, Director, LPA toured the facility inside and outside. A census is taken. The purpose of today's visit is to discuss an Unusual Incident Report (LIC 624B) that occurred on September 25, 2023, when staff one (1) removed child one (1) from the facility and took child, to their personal residence. Following a separate incident that occurred on August 18, 2023 Director submitted their plan of correction stating that ALL staff were trained on updated dismissal protocols on August 23, 2023. The Director provided CCL with updated Bret Harte Preschool Dismissal Procedures, which indicates: 1. Only a lead teacher will dismiss children to their parent/caregiver . 3. Check the ID of any parent/caregiver who is picking up a child, and confirm the identity with the information in their binders. This is to be done every day even when you have become familiar with the pickup person. Also on September 13, 2023, an Informal Office Meeting was conducted at the Fresno South Regional Child Care Office. During the informal meeting, the Director reiterated the facility\342\200\231s sign in/out procedures as declared above. LPA reviewed Sign In and Sign Out sheet for class G3 for the week of September 25, 2023, LPA observed that staff one (1) did not sign out the child nor was she authorized to take the child off grounds. Licensee is informed that the facility is being cited for Sign In and Sign Out procedures specifically failing to implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. Per California Code of Regulations, Title 22, Division 12, Chapter 1, a deficiency is cited. \302\267 An exit interview was conducted with Linda Kroff-Reis. \302\267 Appeal Rights were discussed and issued. \302\267 A Notice of Site Visit form shall be posted to parent's board and must remain posted for 30 days.
