What the inspector wrote

On 02/18/2026, Licensing Program Analyst (LPA), Christopher Burnias met with Site Supervisor, Shoua Moua for an unannounced Case Management incident inspection. LPA toured the facility, and a census was taken. The purpose of today's inspection was to address an unusual incident that was reported to the Fresno Community Care Licensing (CCL) office. LPA interviewed Staff 1 (S1), and Staff 2 (S2) who were present during an Unusual Incident that occurred at the facility. Information obtained from interviews indicate that on 01/21/26 at or around 2:30 PM at pick up time, Child 1 (C1) and Child 2 (C2) were running in the outdoor grass area racing one another when C2 ran in front of C1 causing C1 to slip and hit their face against a wooden crate resulting in slight bleeding on their bottom lip . S1 observed the incident occur and both S1 and S2 cleaned and treated C1’s wound. C1’s parent was present at the time of the incident as they were preparing to check C1 out of the facility. Parent took C1 to be medically examined. Medical staff did not identify major injury, however, C1 received an ointment for the cut on their lip. Site Supervisor informed LPA that the wooden crate C1 bumped into contained children’s play logs (Lincoln Logs) and since the incident, the wooden crate has been removed. Site Supervisor stated that at the time of the incident, there were 2 teachers and 2 children present. Site Supervisor informed LPA that staff will discuss safety expectations with children before they go outside to the outdoor play area. Site Supervisor also stated that staff will verbally guide and redirect children when running too close to each other. After interviewing staff members about the unusual incident, LPA has determined that adequate supervision was provided despite the incident occurring. Staff took appropriate measures to assess the incident, and no regulations were violated. LPA inspected the outdoor play area where the incident occurred and did not observe any hazards present. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited during today's visit. An exit interview was conducted with Site Supervisor, Shoua Moua and appeal rights were provided. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and is required to be posted for 30 days.
