What the inspector wrote

On October 27, 2025, Licensing Program Analyst (LPA) Paul Garcia and Nohemi Sanchez conducted an unannounced Case Management - Deficiency inspection to issue a deficiency found during a Complaint Report investigation received on October 22, 2025. Licensee Gloria Madera\342\200\231s primary language is Spanish and Nohemi Sanchez who is a State certified Spanish interpreter provided interpretation. LPAs met with licensee Gloria Madera and explained the purpose for the inspection. Surveillance observations and photographic evidence collected on October 24, 2025, revealed that a one-gallon plastic container of Dex-Cool 50/50 Antifreeze, a known toxic substance that creates Kidney failure, permanent brain damage and even coma or death. Surveillance observations and photographic evidence collected on October 24, 2025, revealed that a one-gallon plastic container of Dex-Cool 50/50 Antifreeze, a known toxic substance was in plain sight in her vehicle and within reach of child C1. This occurred while Gloria left C1 unsupervised in her parked vehicle with the known toxic substance as she picked up children from Stratford Elementary School. Exposure to this chemical can result in severe health consequences, including kidney failure, permanent brain damage, coma, or even death. The presence of this accessible poison posed an immediate and serious health and safety risk to C1. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is cited on LIC809-D. LPA informed Gloria Madera that this report dated October 27, 2025, documents two (1) Type A citations which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. LPA Garcia also informed Licensee, Gloria Madera she shall provide a copy of this licensing report dated October 27, 2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) shall be placed in each child's file for verification. An exit interview was conducted with Gloria Madera. A Notice of Site Visit form was issued and shall be posted to parents\342\200\231 board and must remain posted for 30 days. Gloria was provided with a copy of her Appeal Rights.

    Operation of a Family Child Care Home 102417(a) – The licensee shall be present in the home and shall ensure that children in care are supervised at all times. On October 24, 2025, and multiple occasions, child was left alone in a running vehicle, with the windows rolled up, while licensee walked across the street, to talk to other adults, while waiting for school to dismiss the other three older school age children. This requirement was not met as evidenced by LPA’s observation and licensee’s statements. This is an immediate health and safety risk to the children in care. An immediate civil penalty has been assessed.
