On November 19, 2024, Licensing Program Analyst (LPA) Paul Garcia conducted an unannounced Case Management Incident inspection. LPA met with Site Supervisor Alejandra Robles. LPA Garcia toured the facility indoors and outdoors and took a census. The purpose of today’s inspection was to discuss an unusual incident that was reported to Community Care Licensing (CCL) on October 17, 2024, where Child 1 (C1) was climbing down a play structure and as she was going down missed a step and fell to the ground causing an injury to the right elbow. Site Supervisor Alejandra escorted C1 into the classroom and provided her with an ice pack and conducted a visual of C1 elbow. Site Supervisor observed the elbow to be warm, red, and swollen. C1 mother was immediately notified at approximately 1:05PM. C1 was picked up by mother at 1:26PM and taken to the hospital for an evaluation. C1 was evaluated at Valley Children’s Hospital. It was determined C1 sustained a closed supracondylar fracture of the right humerus, C1 was initially provided with a split and was released with restrictions of all physical activity (Sports, Recess, PE) with a follow up appointment to receive a cast on the injured arm on November 1, 2024. (Date of restriction from November 1, 2024 - December 15, 2024). LPA interviewed staff and learned new shoes may have been a contributing factor for the injury. Per interviews, LPA determined adequate supervision for the number of children on the play yard was maintained per regulation. Due to this incident, SS has implemented an informal action plan with child’s parent, who both agreed child will not wear that style of shoe while in care. LPA determined all staff at this facility took appropriate actions and that this was an isolated incident. This facility will continue to provide additional training to all their childcare staff to reenforce staffs knowledge in recognizing footwear safety before use of the play structures. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. A notice of site visit was issued and must remain posted for 30 days.
