What the inspector wrote

On May 15, 2026, an Informal Office Meeting was conducted at the Fresno South Regional Child Care Office. In attendance were Director Mai Kue Chang and Site Supervisor Concepcion Carrillo and Executive Officer Violet Chuck. Also present were Licensing Program Manager (LPM) Cynthia Brannon and Licensing Program Analyst (LPA) Paul Garcia. The purpose of this meeting was to discuss a supervision violation of a Title 22 regulation. This violation, if not corrected, poses an immediate and potential risk to the health, safety, and personal rights of children in care. Issue/Violation Discussed Type A Deficiency – Cited March 18, 2026 Regulation Cited: 101229(a)(1) – Care and Supervision Interviews and a review of Audio-Visual Surveillance System (AVSS) footage revealed that facility staff failed to ensure that Child 1 (C1) was under the direct supervision of a teacher at all times. C1 was discovered unsupervised, inside a caterpillar-tunnel sensory play structure located on the play yard. When located, the child appeared scared and emotional. AVSS footage documented that C1 was without required adult supervision. Corrections for all deficiencies were submitted and accepted by the Fresno South Regional Office. The Technical Support Program (TSP) was discussed and recommended, and the licensee agreed to participate. A referral will be submitted. It was discussed that continued violations of Title 22 Regulations and failure to maintain compliance will result in a Non-Compliance Conference and may be referred to the Legal Division for possible Administrative Action. Today no violations were cited during todays meeting. A copy of this signed report was provided to the licensee.
