What Happened

Case management investigation of two incidents involving sibling children revealed inadequate outdoor supervision on 2/24/2026 allowed child's thumb to be pinched in rotating seesaw; separate minor head injury from fall on 2/23/2026 determined to be isolated.

Observations

  • Staff member left outdoor supervision to assist child with toileting, leaving one staff member responsible for approximately nine children
  • Child did not comply with verbal redirect away from rotating seesaw center
  • Five children using device designed for four children
  • Staff aware of prior overcrowding issues on rotating seesaw but did not take active steps to prevent reoccurrence
  • Child's left thumb sustained two small lacerations and visible injury from being pinched between teeter-totter arms
  • Child transported home for medical evaluation and treatment
  • Head injury incident (2/23) involved child tripping on rug and striking closet door/drawer, resulting in quarter-sized bump on forehead
