On 04/09/2025, Licensing Program Analysts (LPA), Sebastian Phouthavong made an unannounced Case Management visit in response to a self-reported Unusual Incident Reports (UIR). The UIR were notified to Community Care Licensing on 06/25/2025. The incident were regarding a child running while at the facility and falling, leading to injury. Previously the department was notified of two separate UIRs regarding the same type of injury on 04/01/2025 & 04/04/2025. LPA met with Licensee, Julie Blevins and spoke with her the purpose of today's visit. During today's inspection, LPA toured the facility inside and out and observed 11 daycare children being supervised by Licensee and assistant. Licensee reviewed to ensure proper supervision when outdoors and to ensure a safe environment for children. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Julie Blevins. There were no Title 22 deficiencies cited during today's inspection.
