Facility-requested visit·June 4, 2026

On 06/04/2026, Licensing Program Analyst (LPAs), Sebastian Phouthavong and Ivet Zamora Perez made an unannounced Case Management visit and met with Facility Representative, Adriana Mendoza and explained the purpose for today's visit. The inspection is to review that the health, safety, and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. The facility is requesting to change the Classroom 3 from a Toddler Classroom to an Infant/Toddler Classroom. The facility is not changing its max capacity. During the inspection, LPAs toured the facility in & out including Classroom 3. The facility’s operating hours are 5:00am-5:00pm, Monday-Friday. This is an E Center, Head Start program. There is a toddler component associated with this infant license. The program operates April through November. The fire clearance was granted on 06/02/2026 by the Kelseyville Fire Protection District. LPAs toured and measured the classroom and observed that the Infant & Toddler contain their own indoor spaces. LPAs also observed that Classroom 3 to has age appropriate toys and equipment and Facility Representative reviewed to ensure both age component do not commingle at all times. The indoor total measurements contain the required square feet to accommodate facility’s request for 4 Infant and 4 Toddler children for classroom 3. The facility has been approved for the change. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Facility Representative, Adriana Mendoza There were no Title 22 deficiencies cited during today's inspection.
