On 12/17/2025, Licensing Program Analyst, Sebastian Phouthavong & James Clark made an unannounced Case Management visit to the facility and met with Licensee, Angela Galvani today to verify operation at the facility and to address the facility's addition to a co-Licensee. During the inspection the home was toured inside and outside. The licensee and two assistants supervised 14 children and operating within the licensed capacity and ratio requirements. During the inspection Licensee reviewed the required forms to add a co-licensee. LPA also received a LIC 279 Application from Licensee and requested additional forms make the change. Additionally, Licensee reviewed the requirements regarding her Capacity limitation and was provided 102416.5 Staffing Ratio and Capacity A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee , Angela Galvani. There were no Title 22 deficiencies cited during today's inspection.
