On 07/23/2025, Licensing Program Analysts (LPA), Sebastian Phouthavong made an unannounced Case Management visit to review the facility procedure when supervising daycare children at the front yard/outdoor area. During today's inspection, LPA toured the facility inside and out and observed 7 children being supervised by Licensee. Licensee stated when they daycare children are playing at the front yard, she is always with them outside. Licensee further stated she haves view of the front yard from the inside the kitchen and childcare room. If Licensee need to step inside the facility, Licensee stated daycare children would accompany her inside. In addition, Licensee stated an assistant is often on site to assist when supervision and the facility has surveillance cameras on site. During the visit, Licensee reviewed to enure proper supervision is being applied when the daycare children are outdoors. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Rachael Stultz. There were no Title 22 deficiencies cited during today's inspection.
