Facility-requested visit·January 9, 2026

On 01/09/2026 at 11:30AM, Licensing Program Analyst (LPA) Sebastian Phouthavong arrived to conduct a change of location pre licensing inspection. An application was received to the department on 12/23/2025 for the change of location; previous license number: 173010746 . An approved fire clearance was conducted 01/07/2026. Services will be available Monday \342\200\223 Friday, 7:30 AM \342\200\223 5:30 PM. The licensee understands that 24hr consecutive care is prohibited. There are currently three adults living in the home. During the inspection the home was toured inside and outside. The floor plan submitted by the licensee was reviewed and verified. The children will have access to the 1st floor of the home that incudes the foyer, 2 childcare rooms, kitchen ,one bathroom, and backyard. The off-limits incudes the 1st floor garages, and entire 2nd floor which is the main living space that incudes, a living room, two bedrooms, two bathrooms and patio. The off-limits areas of the home were made inaccessible by door locks, plastic doorknob covers and/or child gates. There are poisons in the home's garage and has met requirements as of today's visit. The regulation that poisons are to be locked using a key or combination lock was reviewed. There is a functioning smoke detector and carbon monoxide detector and fire extinguisher is rated at 2A10BC. First aid supplies will maintained at the facility at all times. The home's backyard is not fully fenced, however, the entire area is enclosed with a fencing. Licensee reviewed to ensure proper supervision is being provided. There were no pools or other bodies of water observed. The facility will be approved for licensure effective, January, 12 2026.
