What the inspector wrote

An unannounced case management inspection was conducted today 04/08/25 at 12:28am by Licensing Program Analyst (LPA), Sydney Sims. In regard to an unusual incident received at the department on 3/20/25, where Child C1 fractured their femur on an indoor trampoline on 3/18/25. The Director Nicolette Swain was interviewed on at 4/08/25 at 12:39pm and confirmed that the incident did in fact occur but that proper supervision was being provided and that the trampoline was being used up to the manufacture guidelines One Staff S1 was interviewed on 4/8/25 and stated that C1 was jumping alone on the trampoline following the manufacture guidelines and that proper supervision was being provided for C1. One Parent was interviewed on 4/8/25 and stated that C1 was being properly supervised by staff and that P1 believes the facility was utilizing the trampoline up to manufacture guidelines. During today\342\200\231s inspection, the facility was toured, and LPAs observed 6 children in care. Based interviews conducted and information obtained the Facility provided proper supervision and utilized equipment up to manufacture guidelines. There were no deficiencies cited during today\342\200\231s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee.
