What the inspector wrote

An unannounced case management inspection was conducted today at 9:52am by Licensing Program Analyst (LPA), Sydney Sims. LPA met with Site Supervisor Mayra Puente. In response to an unusual incident report received by the department on 02/05/26 where on 02/03/26 at approximately 10:00am there was an inappropriate interaction between child C1 and C2. The Site Supervisor was interviewed on 03/12/26 and stated that on 02/03/26 at around 10:00am that there was an inappropriate interaction between C1 and C2 where C1 and C2 were engaged in a inappropriate conversation. Site Supervisor stated that S1 and S2 were out on the playground and that S1 intervened in the conversation and separated the children. Site Supervisor stated that proper supervision and ratio were being maintained. Two Staff (S1 -S2) were interviewed on 03/04/26 and S1 stated that S1 was told by child C3 that C1 and C2 were having an inappropriate conversation, S1 then walked over to C1 and C2 and separated the children. S1 stated that S1 could see the children but could not hear their conversation, and when S1 went over to the children S1 did not see any inappropriate physical interactions occur. S2 stated that S2 was on the play ground with the children and spoke with C2 about the incident but did not actually observe the incident occur. S1 and S2 stated that proper supervision and ratio were being maintained. LPA Sims obtained copies of the Children's sign in and sign out sheets and observed that there was 14 children present and 3 staff present, the facility was meeting ratio requirements. Although an inappropriate interaction did occur between child C1 and C2, it could not be determined that a regulation violation occurred. Exit interview conducted and report was reviewed with the Site Supervisor Mayra Puente. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
