What the inspector wrote

An unannounced case management inspection was conducted today at 12:03pm by Licensing Program Analyst (LPA), Sydney Sims. LPA met with facility representative Georgina Johnson. In response to an Unusual Incident Report received by the Department on 08/18/25. Where child C1 was playing outside on the play ground and jumped forward off the end of the slide and broke C1's left arm when landing. The facility representative was interviewed on 10/29/25 at 12:20pm and stated that on 08/8/25 at 9:52 am staff S1 was on the play ground with children and child C1 jumped off the bottom of the slide and landed arms first resulting in left arm being broken. One parent (P1) was interviewed on 9/19/25 and stated that P1 did not think there was any issues with supervision or the play ground equipment that C1 was using. P1 stated that P1 does not believe the injury could have been prevented. One staff (S1) was interviewed 10/29/25 and stated that S1 was present for the incident and stated that the children were playing tag on the play ground and that C1 jumped off the side of the slide right before the bottom and landed arms first resulting in a broken arm. S1 stated that S1 was in ratio, providing proper supervision and that S1 provided first aid to C1 after the injury by providing child C1 with a sling to support the arm. On 10/29/25 the facility was toured and the LPA observed the slide that C1 jumped off and found the equipment appeared to be age appropriate. LPA Sims observed there was adequate rubber bark cushioning under the slide. On 10/29/25 LPA Sims reviewed S1's staff record that showed S1 is a fully qualified teacher and is allowed to be left alone with children. LPA Sims reviewed sign in sheets that showed that facility was in ratio. There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with the facility representative Georgina Johnson . Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days.
