Five staff (S1 – S5) were interviewed on 3/20/25 and 6/10/25 and denied the allegation stating that staff do not leave children unsupervised and provide constant supervision. S1 – S5 could not recall a time that children had ever been left unsupervised. Three children (C1 – C3) were interviewed on 3/20/25 and C1 – C3 denied the allegation stating that children are never left alone at the facility and that a staff is always present with the children while in care. During today’s inspection, the facility was toured and LPA Sims observed 26 children in care. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. Exit interview conducted and report was reviewed with the licensee Lori Furtado. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
