What the inspector wrote

An unannounced case management inspection was conducted today at 12:10pm by Licensing Program Analyst (LPA), Sydney Sims. LPA met with Director Stephanie Reid. In response to an Unusual Incident Report received by the Department on 10/20/25, where child C1 was laying underneath a fire extinguisher and kicked the fire extinguisher off the wall causing the fire extinguisher to hit C1 in the head leaving a cut that resulted in stitches. The Director was interviewed on 11/14/25 at 12:17pm and stated that on 10/16/25 @ 12:15pm while transitioning to nap time child C1 was on the ground and kicked the fire extinguisher off the wall and it hit child C1 in the head resulting in C1 needing two stiches. Director stated that first aid was given and that C1's parents were called immediately. Director stated that the facility has had fire extinguisher in that location since the facility was licensed. One child (C1) was interviewed on 11/14/25 and stated that C1 kicked the fire extinguisher off the wall and that it hit C1 in the head. C1 stated that C1 was getting ready for nap time when the incident occurred. During today’s inspection, the facility was toured and LPA observed where the fire extinguisher was installed and observed that fire extinguisher was removed from that installation hook. LPA observed that the fire marshal had serviced the fire extinguisher in April of 2025. Based on observation of the fire extinguisher location and service history, record review of the incident report and interviews conducted although C1 did sustain an injury while in care it could not be determined that a regulation violation occurred. There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with the Director Stephanie Reid. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
