What the inspector wrote

On 10/29/24 ,at 08:30 am ,Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management follow up visit regarding the incident reported to the Department. Upon arrival, LPA met with Facility Representative , Lisa Castaneda , to whom the reason of the visit was explained. Census was taken. There were 32 children and 4 staff members. On 08/19/24 an unusual incident report was submitted to the Department regarding child having an allergic reaction on the facility grounds .During the investigation LPA talked to the Director, staff members , parent. Facility observed to be nut free facility , LPA observed signs on the classroom doors and in the kitchen . Per Director and staff , parents are notified upon enrollment and reminded every time snacks are provided . Per staff members, snacks donated by the parents , verified before served to the children .Menus provided to the parents ahead of time. On the day of the incident ,per staff members ,C1 came in with the snack provided by the parent and was allowed to sit at the table outside until finished which was confirmed by the C1 parent. Per staff member , after C1 finished the snack headed to play. S3 noticed C1 laying on the floor and was lethargic.Per staff members , paramedics were called immediately, medication was administered, and parent was notified .Child was taken to the hospital by the parent and was back to the facility next day . Based on the information obtained at this time, there is not a preponderance of evidence that shows that the facility was in violation with Title 22 Regulations when these incidents occurred. Therefore, there are no deficiencies being cited. Exit interview conducted with Facility Representative ,Lisa Castaneda and Appeal Rights explained. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.
