What the inspector wrote

On 07/15/2025, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced Case Management \342\200\223 Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 06/30/2025. Upon arrival, LPA met with Director, Norma Montes , and explained the purpose of the visit. LPA observed 31 children in care, being supervised by 5 staff. According to the UIR, on 06/27/2025, Child was climbing an outdoor play structure located on the sandbox, fell and landed on left arm . Parents were notified and per parent and a same day appointment was made with the orthopedic. Fall resulted on a fracture located between shoulder and elbow. Child returned to school 06/30/2025 with arm in a sling until arm heals. During today\342\200\231s inspection, LPA conducted interviews, toured the facility both indoor and outdoor and obtained pictures of the outdoor play area. Per Director, climbing structure was removed from the sandbox and replaced with a play house to prevent future incidents from occurring. At this time, no further investigation is needed. An exit interview was conducted with Director, Norma Montes . A copy of this report (LIC 809) and Notice of Site Visit were provided and must remain posted for 30 days.
