What the inspector wrote

On 04/30/2025 at 08:25 AM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced 3 Year Annual Required Inspection. Facility is located on 12227 Balboa Blvd., Granada Hills, CA 91344. LPA identified self and met with Sandra Valdivia, Assistant Director, LPA identified reason for visit and provided the Entrance Checklist -Child Care Centers (LIC 125) to help facilitate the inspection. Mary Kandilian, Director arrived shortly after. Sandra Valdivia, Assistant Director allowed entry into the facility and provided LPA with a guided tour of inside and outside of the facility. The facility is currently licensed for a capacity of 77, per FR they currently have 73 enrolled. There were 64 children and 7 staff upon arrival. The facility hours of operation are currently Monday thru Friday from 07:00 AM to 06:00 PM. Required postings were observed posted in the school lobby. The facility is located on the same property as Bethlehem Lutheran Church 12227 Balboa Blvd, Granada Hills, CA 91344. The facility is a one-story structure. There are five (5) Preschool Classrooms; Preschool #1 Classroom ages are 2 year-olds, with 2 staff, Preschool #2 Classroom ages are 3 year-olds, with 1 staff, Preschool #3 Classroom ages 3 year-olds and 4 year-olds, with 1 staff, Preschool #4 Classroom ages 3 years-olds and 4 year-olds, with 1 staff, and PreK Classroom ages 4 years-olds, with 2 staff. The facility includes three (3) Infant Classrooms License number 197402147 and one (1) School Age Classroom License number 197420045. PAGE 1 OF 4 Beginning at 08:40 AM, LPA observed the following in the Preschool and PreK Classrooms. The furniture, toys, and equipment were inspected for age appropriateness and were in good repair. There were individual mats for each child. There were cubbies used for each child to store their belongings including sleeping linens. Drinking water was readily available with the use of individual water bottles labeled with the child\342\200\231s name and filtered water was available inside the classroom or in the classroom hallway from a filter dispenser for water refills. LPA observed two (2) restrooms available for children to used. There were nine (9) toilets and nine (9) sinks available for children to use and were safe and sanitary. The facility has a telephone service via a landline, heating, lighting, and ventilation were evaluated for safety and comfort. LPA observed a first aid kits located each classroom and were inventoried for necessary supplies . LPA observed in each classroom smoke detector and carbon monoxide detector. Disaster drill log was available, and the last disaster drill was conducted on 04/09/2025. Per Assistant Director, isolation area for sick children is the office. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed cleaning solutions stored in cabinets with a child safety latch or a cabinet door lock. LPA did observe a kitchen area that is used to prepare snacks. Per the Assistant Director, snacks are provided, and meals are provided by an outside third-party vendor. Beginning at 09:05 AM, LPA toured the outdoor play area and observed playground equipment is in a safe condition, free of sharp, loose, or pointed parts, in good repair and age appropriate. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Children have access to a climbing structure located in the play area, surrounding and under the structures there is a wood chips to absorb falls. There is adequate shade in the play yard. The children have access to their individual bottles of water, refills are provided with a filtered water dispenser located in the outdoor play area and is used to refill drinking water during outdoor play. LPA advised that no children shall be left without the supervision at any time. There is no swimming pool or other bodies of water on the premises. PAGE 2 OF 4 Beginning at 01:15 PM, staff and children\342\200\231s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires on 06/2025 for Director. Inspection of required forms was discussed. LPA observed that teacher-child ratios were maintained, and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed children signed in using an online application at time of inspection. Personal Rights of children were observed by LPA. This facility provides Incidental Medical Services - IMS. LPA reviewed storage of "medication and equipment/supplies, and reviewed children's, personnel, and administrative records. For IMS information see PIN 22-02-CCP. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. PAGE 3 OF 4 For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Mary Kandilian, Director and Sandra Valdivia, Assistant Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
