Incident investigation·April 16, 2026

On 4/16/2026, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Director Miriam Mancilla who guided LPA on a tour of the facility. LPA explained the purpose of today's visit to follow-up on an incident which occurred on 4/13/2026. LPA observed the following classroom ratios: Sunflower Room ( 2 Teachers, and 10 children); Ladybug Room (1 Teachers, 1 Teachers Assistant and 7 children), Dragonfly Room (2 Teachers, and 11 children), Bumble Bee Room (1 Teachers, 1 Teachers Assistant, and 10 children), Grasshopper Room (2 Teachers, and 10 children), Hummingbird Room (2 Teachers and 12 children). The incident that occurred on 04/13/2026, was reported to the Department on 04/13/2026, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The written report has been received to the Regional Office on 4/15/2026. The incident which occurred a tree was removed during Spring Break due to root rot. LPA interviewed Director, took photos of physical plant and obtained other pertinent information. Page 1 of 2 Per Director tree was removed out of precaution. Facility has since purchased shade sails to provide shade for children. Per Director they will temporarily use the shade sails until facility is closed for break from 6/08/2026 to 6/13/2026. LPA was informed that is when construction will begin during those closure dates, for the single post Cantivelier umbrella inground fixture. There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Miriam Mancilla.
