On 11/06/2025, at 2:25 pm , Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management Inspection to follow up on the incident that took place on 10/30/2025. LPA met with an Assistant Director Shannon Macall, who guided LPA on a tour of the facility. Census was taken. There were 44 children and 7 staff members. On 10/30/2025 an unusual incident report was submitted to the Department regarding child sustaining an injury on the facility grounds .The Unusual Incident/ Injury Report was reported as required. During this visit , LPA toured the facility with the Facility Representative . Per Facility Representative and injury report , incident occurred in the bike yard . LPA toured the bike yard and observed cracks and uneven pavement where incident took place . (Pictures were taken) LPA obtained incident report (ouch report) prepared by the facility, sign in and out sheets for children and staff members, interviewed staff member present during the incident. LPA obtained information that 10 children were present with 2 staff members when incident occurred . Per staff member child #1 was running when child #1 shoe became lodged in a crack making child trip and fall . Child sustained broken arm and full arm cast was placed. Page 1 of 2 Per Facility Representative, bike yard will not in use until cracks are filled. Additionally , facility representative stated that facility plans new pavement for the yard either end of this year or beginning of the new year. Children will temporarily use basketball yard and courtyard on a staggered schedule until bike yard is safe to use. Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (Deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be clear to protect the children’s health and safety. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. An exit Interview was conducted, a copy of this report along with Notice of Site visit , Deficiency pages and Appeal Rights were explained and provided to the Facility Representative , Shannon Maccall on 11/06/2025.
