What the inspector wrote

Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management inspection for the purpose of following up on an unusual incident report. Upon arrival at 1:00PM, LPA met with Director Arpy Jahjah and assistant Director Shannon Macall. Tour of the facility was provided. There were 49 children and 8 staff members present during the inspection. On 04/09/2026, an unusual incident report was submitted to the Department regarding a child sustaining an injury on the facility grounds. Based on the information submitted, child tripped over their feet and fell , hitting their mouth on the changing table. Based on the sign-in sheets provided by the facility there were 7 children present with 4 staff members the date of the incident. Per staff member observing the incident , C1 went over the table and tripped over on his feet hitting his mouth on the diaper changing table. Staff disclosed, first aid was provided immediately and parents were notified. During this visit, LPA obtained incident report .LPA toured the area and did not observe any tripping hazards in the area where the incident occurred. Per staff member, children were often reminded to use walking feet. Based on the information obtained, LPA determined that there was adequate supervision on the day of injury, no deficiency cited during today's visit. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
