What the inspector wrote

On Wednesday, May 28, 2025, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management-Incident inspection and met with director Dana Ahern who granted access to the facility. During this visit LPA observed 6 preschool children present with Staff #1 present providing care and supervision. Palmdale Regional office received a self reporting unusual incident on May 8, 2025, stating a child broke their arm when the child slipped and fell as the child tried to show a teacher the jump from the playground steps. LPA inspected the outdoor playground and playground structure. LPA observed the playground structure to be age appropriate and anchored. LPA was not able to interview child or staff who observed the incident because the staff is no longer employed. The interviews conducted with director and staff 1, indicated the incident was an accident and the incident did not occur due to lack of supervision. At this time further follow up is not needed and no deficiencies are being cited during this visit. An exit interview was conducted and a copy of this report was provided to the Director, along with Notice of Site Visit.
