What the inspector wrote

On 08/01/2024 at 9:30 am, Licensing Program Analysts (LPAs), Crystal Green and Shushanik Safaryan conducted an unannounced Case Management inspection for an increase in capacity. LPAs met with Facility Representative Patrica Chambers. This facility is currently licensed as an infant program with a capacity of 20 children serving ages 6 weeks to 24 months in (2) classrooms: Classroom 106 serves ages 12 to 24 months and classroom 207 serves ages 6 weeks to 12 months. The Licensee, La Crescenta Presbyterian Church Center for Children, has requested to increase the infant program capacity by adding an additional classroom to their license, classroom 101 which will be designated for ages 12 to 24 months, this change will increase the capacity by 14 children for a new total infant capacity of 34 children. Also operating on the premises is a preschool with a toddler option program (191201896) licensed for a capacity of 111 children (99 preschoolers and 12 Toddlers) in classrooms 102, 104, 105, 107, and 108 and a school-age program (191223032) licensed for 60 children in classroom 201, 202, and 204. The toddler option program operates out of classroom 102. Hours of operation will be Monday to Friday from 7am to 6 pm. Present during today\342\200\231s inspection were 12 children with 4 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Patrica Chambers that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPAs continued to tour the facility and measured new indoor activity space. Total new indoor activity space (classroom 101) measured for 435.28 square footage, which is sufficient to accommodate the requested increase of capacity. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filter water jugs along with children\342\200\231s personal drinking bottle. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 4/25/2024. LPAs observed classroom 101 to have a total of 2 sinks and 1 toilet available for children\342\200\231s use. These are sufficient to accommodate the requested capacity. The infant room have at least one potty chair or toilet for every five infants. These are sufficient to accommodate the requested overall capacity of children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the main office area. This facility currently has a (2) Outdoor play areas designated for the Infant Program. The total square footage for infant outdoor activity space is 3,041.00 square footage, which can accommodate the requested capacity. Shade is provided via fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via the child\342\200\231s personal water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Facility Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The following corrections are needed prior to the issuance of the license: Licensee will need to make classroom 101 age-appropriate by removing the unused furniture and storage equipment in order to provide care to children. Licensee will need to clear the bathroom area located within classroom 101 so it that can be accessible to children. Licensee will need to obtain a sound chime door alarm on the second entrance located near the rear of the classroom. Licensee Representative Patrica Chambers understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Once the corrections received and the final file review conducted, the infant program will be licensed to care for 34 infants. Exit interview conducted and report was reviewed with the Licensee Representative, Patricia Chambers.
