What the inspector wrote

An office meeting was held today with Patricia Chambers, Director, to review the facility's history, discuss substantial compliance with licensing laws and regulations, and provide resources to the licensee. In attendance were Licensing Program Analyst (LPA) Shushanik Safaryan and Licensing Program Manager (LPM) Brandi Van Oosten. The following California Title 22 Regulations deficiencies were reviewed: Responsibility for Providing Care and Supervision, Reporting Requirements. 101229(a)(1) Responsibility for Providing Care and Supervision. (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. -During summer of 2023 , a child was left unattended inside of the vehicle (van) after the field trip. The facility was cited a Type A violation on August 2 nd , 2024 for this incident. 101212(d)(1)(C) Reporting Requirements. Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. - During the summer of 2023 a child was left unattended inside of the vehicle (van) after the field trip. The facility was cited a Type B violation on August 8, 2024, for not reporting this incident to the Department. Page 1 of 2 To ensure compliance, the implementation of corrective and preventative measures was discussed. Director stated when incident happened immediately after the incident meeting was conducted with all drivers , in all the staff meetings facility mentioning head count, when they conduct pick up and drop off. Facility have sign in sheets where they check off the children , verify children present. Staff will head count children before leaving the classroom and entering vans , another head count will be conducted when children will exit the vans and entering the classrooms. Facility staff will inspect inside of the vehicles after each use to ensure no children left inside. Per Director , whenever she is present she will conduct spot check , to make sure staff following to the safety requirements . Director stated , she reviewed with 2 other staff members reporting requirements and will make sure incidents reported as required per regulations. The licensee was informed of the following: 1. Inspections of the facility will be increased to ensure compliance. 2. Responsibility to comply with Title 22 Regulations. 3. The licensee was advised to check the Childcare Licensing web site at www.ccld.ca.gov for quarterly updates, forms, and regulations. 4. The licensee was advised to attend local licensing meetings and subscribe to provider updates. 5. The licensee was advised to attend Step 2 Orientation. In-service training was discussed. Technical Support Program (TSP), Active Supervision At-A-Glance, and a list of CDSS videos were provided to the licensee. E xit interview was conducted with the licensee, Patricia Chambers , who agreed with the topics discussed during today's meeting. Page 2 of 2
