What the inspector wrote

On August 15, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Director, Arpy Jahjah, who guided LPA on a tour of the facility. This is a School aged program that operates Monday – Friday. Currently the facility is providing all day care as children are on summer break. Hours of operation are from 7 am – 6 pm. During the school year hours of operation change to 1:00 pm – 6:00 pm. Per Director, there are 67 children enrolled in the program. LPA observed 29 children in care and 4 staff members. The purpose of this inspection is to follow up on a self-reported incident of hand, foot and mouth that occurred at the facility on 08/12/2025. Brief summary of incident: On 8/12/2025, the mother of Child #1 (C1) came to the facility to drop off her daughter to the program and informed the director that her son will not attend camp today as he woke up with some unusual spots on his hands. The mother informed the director that she will take C1 to see his physician. Later that day the mother called the facility to inform them that C1 has a confirmed case of Hand, Foot, and Mouth. Per director, C1 participated in room 201 and the facility sanitized all tables, chairs, the coach and surfaces that children and staff touch on 08/12/2025. The child has been cleared to return to the facility on 8/18/2025 if no other visible symptoms arise. The facility did communicate will all parents who participate in the school aged program about the exposure. Page 1 of 2 During this inspection, LPA obtained proof of parent communication about the exposure, C1 doctor’s note and notice of exposure that was posted at the front office of the facility. The facility has taken all precautionary measures to ensure the health and safety of the children in care. LPA did provide the director with a handout about Infectious Disease for future outbreaks. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of this report was provided to Director, Arpy Jahjah.
