What the inspector wrote

On August 15, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Director, Arpy Jahjah, who guided LPA on a tour of the facility. This is a School aged program that operates Monday – Friday. Currently the facility provides all day care as children are on summer break. Hours of operation are from 7 am – 6 pm. During the school year hours of operation change to 1:00 pm – 6:00 pm. Per Director, there are 67 children enrolled in the program. LPA observed 29 children in care and 4 staff members. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 07/09/2025. Brief summary of incident: Child #1 (C1) was playing soccer with friends in the play yard. When C1 went to kick the ball C1 rolled his left ankle. Staff #2 (S2) assisted C1 and provided an ice pack. C1 could not apply any pressure on left foot and was carried to a bench until Staff #1 (S1) came to the play yard to observe the incident. Then S1 and Staff #3 (S3) carried C1 to the office and called C1’s representatives. C1 was taken to see a physician where it was confirmed that C1 sprained left ankle. C1 was ordered to wear a boot for 4 weeks. In the course of the investigation, LPA interviewed Staff #1 (S1) and Child # 1 (C1). LPA was unable Page 1 of 2 to interview Staff #2 (S2) and Staff #3 (S3), as S2 is no longer employed with the facility as S2 is a seasonal employe and S3 is on vacation. LPA toured the play yard area where the incident occurred and did not identify any hazards. Based on interviews and observations conducted there was no corroborating information to determine that there was a lack of supervision during this incident. The incident was deemed an accident. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of this report was provided to Director, Arpy Jahjah.
