What the inspector wrote

On August 15, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Director, Arpy Jahjah, who guided LPA on a tour of the facility. This is a School aged program that operates Monday – Friday. Currently the facility provides all day care as children are on summer break. Hours of operation are from 7 am – 6 pm. During the school year hours of operation change to 1:00 pm – 6:00 pm. Per Director, there are 67 children enrolled in the program. LPA observed 0 children in care as they 29 children are on a field trip along with 5 staff members. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 08/05/2025. Brief summary of incident: On 8/05/25, Staff 1 (S1) transported 7 school aged children to a field trip in the facility van. Staff 2 (S2) was also present in the van supervising children. On their way back to the facility at around 4 pm, the van was at a stop light when another car rear ended the van. S1 pulled over and exchanged information with the other driver, while S2 remained with the children inside the van. All 7 children were examined during the incident and were not hurt. LPA verified that S1 and S2 obtained criminal record clearance. LPA also verified that S1 is a qualified driver for the facility and obtained qualifications to transport children Page 1 of 2 LPA inspected the 10 passenger van that was in the accident and did not identify any hazards that can pose a danger to children while being transported. Per Director, all children’s representatives were notified of the incident by via Brightwheel on 8/05/2025. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of this report was provided to Director, Arpy Jahjah.
