What the inspector wrote

On 05/01/2026, at 2:00 PM, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management Inspection to follow up on the incident that was reported by the facility to the Department . LPA met with Director Arpi Jahjah and assistant director Shannon Macall. Tour was provided. During the tour , LPA observed 5 children with one staff member. More children came in later after the school finished. On 04/08/2026, an unusual incident report was submitted to the Department regarding child sustaining an injury on the facility grounds .The Unusual Incident/ Injury Report was reported as required. LPA observed the area where alleged incident took place.( Pictures were taken). Based on the sign-in sheets provided by the facility there were 26 children and 2 staff members present during the incident. LPA conducted an interview with the staff member present and child involved. Per staff member, they observed C1 running after the ball and collided with the C2 that was on the bike, which was confirmed by C1. Per staff member , play area has a designated area for driving bikes and children are constantly reminded to drive in the designated area. Per staff members’ statements , incident happened very fast and they were unable to intervene. Per assistant director , as a preventive measure children won’t use bikes at this time. Facility will notify Department regarding the procedure and safety measures for sharing outdoor play area for different types of activities. No citations issued on this day. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
