What the inspector wrote

On 05/01/2026, at 1:00pm, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced case management inspection to follow up on the reported incident. Upon arrival, LPA was greeted by Assistant Director, Shannon Macall and Director Arpy Jahjah. Tour was provided. During this visit, LPA observed 5 children with one staff member. On 03/04/2026, Department was notified by the facility regarding the incident involving child`s personal rights. During this investigation, LPA talked to Director, staff members, teacher and children involved. Based on LPA`s interviews and records review, statements were inconsistent and there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. No citations are issued at this time . A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of this report was provided to Assistant Director Shannon Macall on 05/01/2026.
