What the inspector wrote

On April 16, 2025, at 4:25 P.M., Licensing Program Analyst (LPA) Joselito L. Del Mundo conducted a case management inspection at the facility. LPA met with Risk Manager Vincent Lee and stated purpose of the inspection was to review the staff files and children’s files. During this visit, LPA observed 32 preschool school age children with four staff providing care and supervision. LPA conducted an interview with preschool age children and staff. LPA also review staff’s files and children’s files. Based on records review, children’s files are complete. However, staff #4 has missing documents. Type B deficiecies were cited for the missing documents. See LIC809D. Additionally, per Guardian, staff #4 is not fingerprint cleared. Type A Deficiency was also cited during this inspection. A LIC 9213 Notice of Site Visit was left at facility and must be posted for 30 days. Failure to do so will result in an immediate civil penalty assessment of $100.00. An exit interview was conducted, Appeal Rights and a copy of this report were provided to Risk Manager, Vincent Lee.
