What the inspector wrote

On 07/11/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Branch Director Annie Azizian , for a follow up Case Management Incident inspection involving an Incident Report dated 06/27/2025. Upon arrival, LPA observed 22 preschool children in care. Description of the incident : On 06/26/25: Child #1 had a fever. Parent #1 was called to pick child #1 up. While parent #1 was at the facility to pick up child #1, child #1 had a seizure in parent #1 arms. Teacher #2 called 911. Paramedics came and evaluated child #1. Child #1 vitals were normal, and parent #1 took child home. Child #1 does have a history of seizures. Based on documentation obtained and interviews conducted with staff and other relevant parties, it was determined facility took appropriate measures to ensure the health and safety of child #1. Child #1 temperature was taken immediately once child arrived to the facility. Teacher #1 then immediately called Parent #1 due to child #1 had a fever of 102. While child #1 was having a seizure, Teacher #1 placed child #1 on their side to prevent any choking, 911 was immediately called and paramedics arrived at the facility. Parent #1 did not initially report child #1 was prone to seizures on enrollment packets/medical documents, therefore the center was unaware of child #1 seizure activities. Per Parent #1, child #1 was only enrolled at the facility for one week starting 06/23/25 and last day was 06/26/25 for summer program. Please see LIC809-C for continuation page. No deficiencies have been cited at this time. Incidental Medical Services (IMS) plan was also discussed with Branch Director if needed in the future for other children who may be enrolled. Provider Information Notice (PIN) 22-02-CCP Best Practices Related to Health Care Services was also given and discussed to Branch Director. An exit interview was conducted and a copy of this report was read and provided to Director Annie Azizian, as well as her appeal rights and Notice of Site Visit.

Facility failed to notify department of this occurence, which poses a potential health and safety risk to children in care.
