What the inspector wrote

On 07/11/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Branch Director Annie Azizian for a case management-deficiencies report. Upon arrival, LPA observed 22 preschool children in care. While LPA Tamayo was conducting interviews for a case management incident inspection report dated 07/11/25, it was revealed child #1 was diagnosed with Hand, Foot, and Mouth Disease on 06/27/25 after their Pediatric appointment. The facility failed to notify the Palmdale Regional Office of this occurrence, which poses a potential health and safety risk to day care children in care. Facility has been cited a Type B Citation for Reporting Requirements 101212(g)(2)(a). Please see LIC809-D for deficiency page. Facility will notify and send LIC624 to unusual incident report for plan of correction no later than 07/14/25. An exit interview was conducted, and a copy of this report was read and provided to Branch Director Annie Azizian, along with a copy of her appeal rights and Notice of Site Visit.
